Basic Policy
Payment for services is due in full at the time the service is provided in our office. We accept cash, checks and credit cards.
For Patients with Insurance
We bill most insurance carriers for you if proper paperwork is provided to us. We will also bill most secondary insurance companies for you. Co-payments and deductibles are due at the time service. Listed below is the Insurance Plans we are in network.

Fees
All copays, co insurance and deductibles are due at the time of visit. Our office will verify your insurance benefits. If you have no insurance, we would appreciate payment at the time of service.
Surgery Fees
All copays, deductibles and payments for surgical procedures are due prior to your surgery. A surgery cost analysis form will be given ahead of time to let you know the cost of the surgery.
Your carrier may require prior authorization and our office takes care of that for you.
